要旨：術後乳糜胸水・腹水は術中リンパ管損傷に伴う比較的稀な合併症だが，その遷延は重篤な病態を引き起こすため早期の適切な治療が必要となる．乳糜胸水・腹水に対する治療の第一選択は，適切なドレナージおよび食事療法，薬物療法などの保存的治療である．最近は，完全静脈栄養の進歩に伴い，絶食＋TPN管理に加えてソマトスタチンアナログ製剤の投与が有用とされる．しかし，保存的治療が無効な場合は外科的治療も考慮される．リンパ管損傷部位が明らかな場合は同部位の結紮術が有効だが，損傷部位が不明瞭な場合は腹腔-静脈シャントやフィブリン糊散布なども施行される．本稿では，エビデンスに基づいた術後乳糜胸水・腹水の基本的治療方針を述べ，当科における術後乳糜腹水発生症例を検討する．
