術後乳糜胸水の治療方針

要旨：術後乳糜胸に対する治療法についてまとめた．基本的にはドレナージ，絶食，低脂肪食，中心静脈栄養，胸膜癒着術などであるが，1日排液量500～1,000ml以上が持続する場合や食道癌手術後に対しては胸管本幹損傷の可能性が高く，早期外科治療を考慮すべきである．低侵襲である胸腔鏡下手術を早期に行い，乳糜胸長期化による全身状態低下をきたす前に治療することが望ましい．近年報告されているオクトレオチドは効果が期待できる治療であり，今後症例の蓄積が待たれる．
