括約筋切除による肛門温存手術

■歯状線から口側にある腫瘍の肛門側進展はきわめて低率で，挙筋群や外肛門括約筋への浸潤は少ない．

■癌の浸潤が内肛門括約筋を超える（T3，T4）と疑われたら，部分的あるいは広範囲の外肛門括約筋切除（ESR）を行うが，切除後の病理診断では浸潤は小範囲にとどまっていることが多い．

■腹腔側からは直腸壁と肛門挙筋群との境界が不明であるが，腹腔側から1cmでも剥離をしておくと，後の経肛門操作が非常に容易となる．

■結腸肛門吻合は狭骨盤，肥満，分厚い腸間膜，大きな脂肪垂の症例では，腸管がスムースに肛門管を通過せず，圧迫による血流障害が起こりやすく縫合不全の確率が高くなる．
