■開腹および会陰操作では，術野の展開が最も重要である．骨盤内直腸の剥離には，各種の幅および長さの鉤を使い分けて，術野を展開し，剥離を進める層と切離する組織を直視下に視認して操作を進めることが最も重要である．

■骨盤臓側筋膜，自律神経の含まれる尿管下腹筋膜，壁側筋膜，Denonvilliers筋膜，直腸仙骨筋膜，挙筋上腔，perineal body，恥骨直腸筋，neurovascular bundleを解剖のポイントとして，出血の少ない，層に沿った切除を行うことで，癌遺残の少ない手術が期待できる．
