開腹下に挿入された肝動注カテーテルの体表への誘導経路

要旨：開腹下の肝動注リザーバー挿入のカテーテル誘導経路については報告がない．本稿では，当科で使用しているカテーテルの体表への誘導経路を紹介する．カテーテルを胃十二指腸動脈から挿入し，膵頭部前面から小網背側に誘導する．腹部食道脇から小網を貫通し，食道前面を乗り越え，左横隔膜の腹膜下に挿入する．そこから左外尾側に誘導し，肋骨弓下約3cm尾側で腹壁を貫通し，皮下に達し，ポートと接続する．全身化学療法の進歩で肝動注療法が施行される頻度は減少したが，全身化学療法との併用によって良好な治療成績も報告されている．治療継続のためには，より安全で長期間使用することが可能な挿入方法と誘導経路が重要と考えられる．
