大腸癌におけるPET/CT検査の意義

要旨：大腸癌に対するFDG-PET/CT検査は，大きく2つの臨床側面に生かされる．1つは術後フォローアップにおける早期再発診断である．PET/CTのFDG集積による良好な視認性は，従来のCT診断が苦手とした局所再発や腹膜再発の診断能の向上に寄与した．また，術前リンパ節転移診断においては，原発巣よりもやや離れた2群以上リンパ節で良好な診断能を示した．CTやMRIで行われてきたリンパ節径からの良悪性診断には限界があり，SUVのカットオフ値による客観的診断法を加えることにより，診断能は向上しうることが分かった．さらにこのPET/CT情報の3D画像は術前シミュレーションとしても高い有用性を示した．
