出血部位を術前に特定し得た空腸憩室出血の1例

要旨：術前に出血部位を特定し得た空腸憩室出血の1例を経験したので報告する．患者は78歳，男性．2008年6月に吐下血で救急外来を受診した．血管造影検査で第1空腸動脈に造影剤の漏出を認めたため，同部に止血目的にコイリングを行ったが，翌日に再度の下血とショック症状を呈したため，緊急手術となった．術中所見ではコイルによって出血している憩室を特定し，周囲の憩室を含めて十二指腸水平部遠位から空腸の部分切除を行った．空腸憩室出血は出血部位を特定するのが困難であるが，術前に血管造影検査で出血源と考えられる血管にコイリングを行うことで手術を容易に行えると考えられた．
