胆道癌に対するneoadjuvant chemoradiotherapyの試み

要旨：高い局所制御能を有する照射療法を化学療法とともに切除前に行う術前化学放射線療法（NCRT）は，食道癌や直腸癌ではすでに標準治療として定着しており，最近では膵癌でもその有用性が立証されているが，胆道癌ではほとんど行われていない．2007年から導入したわれわれのジェムザール＋体外照射45GyによるNCRTはきわめて強い抗腫瘍効果を示すが，残肝障害が問題であり，2008年以降，線量を36Gyに減量した．NCRTを標準治療とするには未だ解決すべき問題点が多いが，近い将来，予後不良な胆道癌に対する集学的治療の一環となることを期待している．
