進行胆囊癌に対する肝動注化学療法と放射線治療の併用療法によるdown-staging

要旨：進行胆囊癌は，診断確定時には切除不能例が半数以上を占めているのが現状である．胆囊癌の予後を改善するためには，down-stagingを得たのち，根治手術を行うという治療戦略も考えられる．

　しかしながら化学療法単独では現時点では奏効率はそれほど高くはなく，down-stagingが得られる症例には限りがあるが，われわれが施行してきた動注化学療法と放射線療法の併用療法は奏効率も高いため，積極的な外科切除に結びつく可能性がある．

　今後は，さらに新たな抗癌剤や分子標的薬の開発，各種併用療法のレジメンの開発なども進み，より強力な治療戦略が実現する可能性が高い．進行胆囊癌に対するdown-stagingを目的とした抗腫瘍療法が，集学的治療の一環としてますます重要になってくると思われる．
