Indocyanine green（ICG）蛍光観察による肝切除

要旨：Indocyanine green（ICG）は暗緑青色の色素で，肝機能検査で頻用される薬価収載された検査薬である．ICGは励起光によって蛍光を発するが，この蛍光は不可視な近赤外光で生体内透過性が高く，約5～10mm程度の組織を通過し，近赤外光を観察するカメラ装置で観察することが可能である．この特性を利用し，近年，肝区域，胆囊静脈環流領域，胆管の同定や肝切除後の胆汁リークテスト，肝細胞癌や転移性肝腫瘍の同定，生体肝移植，腹腔鏡下手術に応用され始めている．この古くて新しい薬物を用いた，ICG蛍光法によるナビゲーション手術は肝切除術をより安全にする可能性がある．今後，さらなる発展が期待される．
