膵癌に対する腹腔鏡（補助）下手術

要旨：近年の内視鏡外科の驚異（脅威？）的な進歩・普及は，ついに肝胆膵のmajor surgeryにまで及んできた．膵体尾部切除術はもちろん，消化器外科における内視鏡下手術の究極の到達点の1つであると考えられる膵頭十二指腸切除術でさえ，1つ1つの手技を丁寧に行えば可能となってきた．しかし，通常型浸潤性膵管癌にまで適応を拡大するには，膵癌に特有な腫瘍学的な問題や技術的な問題が山積している．これらの問題が解決されたとき，膵癌のminimally invasive surgeryとして腹腔鏡下膵切除が認知されていくことが期待される．
