胆囊・胆管癌治療の相違点

要旨：わが国における胆道癌，特に胆囊癌，肝門部胆管癌の積極的な外科治療は世界をリードしているといっても過言ではない．肝門部の緻密な解剖をはじめ，術前進展度診断，胆管ドレナージによる減黄術，門脈塞栓術，そして手術療法，およびその術後成績など，わが国から数多くの論文が世界に向けて発信されている．欧米でも，わが国の拡大肝葉切除例における胆管ドレナージ，必要に応じての門脈塞栓術などのstrategyを踏襲し，外科治療を行っている施設もあるものの，術前の進展度診断はかなり大ざっぱに行われる傾向にあり，また，進行症例に対する手術適応のcriteriaもわが国と異なり，Stage Ⅲ，Ⅳの外科切除率は低いのが現状であろう．また，もう1つ大きく違うところは欧米の一部の施設において肝門部胆管癌のうち限られた症例に対し，化学放射線療法後に肝移植を行っている点である．しかしながら，移植後の成績はわが国での外科切除成績と比較しても良好とはいい難く，今後の治療成績をStage別に注意深くみていく必要があるだろうと思われる．
