十二指腸乳頭部癌治療の海外との相違点

要旨：十二指腸乳頭部癌は切除率も高く，根治切除により長期生存が十分に望める腫瘍である．その標準治療は2群のリンパ節郭清を加えた膵頭十二指腸切除術である．長期成績に関しては90年代には本邦は諸外国に比し優れていたが，2000年代になって諸外国においても本邦と同等の成績が報告されている．また，リンパ節転移のない早期癌を適応として経十二指腸的乳頭切除術や内視鏡的乳頭切除術などの各縮小手術があるが，いずれもその評価はいまだ議論の余地が残されている．それぞれの治療法に関する評価と適応が確立し，エビデンスに基づいた治療の選択肢が増えることによって，個々の病態に応じた治療が可能になると思われる．
