Components separation法による腹壁瘢痕ヘルニア修復術

要旨：Components separation法は正中型腹壁瘢痕ヘルニアに対する修復術である．外腹斜筋腱膜を切開し，外腹斜筋と内腹斜筋の間を剝離することで腹直筋を正中へ伸展させ，直接縫合する手術法である．この術式の最大の利点は，メッシュなどの人工材料を使用せずに腹壁を再建できることにある．したがって，創部感染合併例や緊急手術例はもちろんのこと，他疾患との同時手術も可能であり，臨床上の適応範囲は広い．修復可能なヘルニア門の横軸長はウエストライン上で約15cmまでと考えられるが，上腹部の肋骨弓付近と下腹部領域での腹直筋の伸展はやや不良である．手技は比較的容易であり，われわれ一般外科医でも十分に習得することが可能な修復術である．
