要旨：内視鏡的粘膜下層はく離術（ESD）全盛時代における当院での外科医のESDへの関わり方や，内科・外科間の連携などを中心に概説した．当院では内科・外科双方で独立してESDを行っているが，適応や切除法などに関しては統一するようにしている．診断や適応に迷う症例は合同カンファレンスで議論し，偶発症が発生した場合やESD後追加外科治療が必要な場合は綿密な連携をとって対処している．ESD後，非治癒切除因子が明らかとなった場合は，予想されるリンパ節転移率などを提示したうえで追加外科治療の必要性を患者サイドに十分に説明し，同意を得たうえで腹腔鏡補助下胃切除術を行っている．また，胃切除拒否例に対しては，リンパ節転移の有無を確定するための全胃温存腹腔鏡下リンパ節郭清術を当科独自の臨床試験として提示し，同意が得られた症例に限ってこれを行うようにしている．
