膵癌の終末期の特徴とターミナルケア

要旨：膵癌は予後がきわめて不良であり，診断時には根治切除が不能であることが多い．癌の進行に伴って出現する症状は苦痛を伴い，quality of life（QOL）が低下して治療の妨げになる．疼痛に対してはWHO方式がん疼痛治療法を正しく適応し，また，可能な施設であれば腹腔神経叢ブロックを考慮する．閉塞性黄疸に対しては胆道ドレナージが必要であり，可能な限り内瘻化を目指すことが重要である．症状緩和を目的として化学療法を行うことにはエビデンスがあり，全身状態を考慮して行うことが望ましい．緩和ケアは苦痛なく手術や化学療法の治療を行うためにも，診断時より治療と並行して行うことが患者のQOLを維持するためにも重要である．
