術前療法の適応

■cStage Ⅱ，Ⅲ（T4は除外）食道扁平上皮癌

　食道癌に対する術前補助療法のメリットとしては，腫瘍縮小効果による根治性の向上，微小転移巣の制御による再発抑制，感受性試験としての意義などが挙げられる．また，手術で血管網が破綻する前，または手術侵襲が加わる前に投与できることによりdrug deliveryおよび完遂率の面でのbenefitも期待される．

　JCOG9204およびJCOG9907の結果より，現時点でわが国における切除可能食道癌Stage Ⅱ，Ⅲ（T4は除外）に対する標準治療は，5-FU/シスプラチンによる術前化学療法2コース後＋手術と考えられている．その他の病期（Ⅰ期，Ⅳ期やT4）に対する術前補助療法のエビデンスは示されていない．
