アルゴリズム（図1）

　胆道癌では外科切除が唯一根治を望める治療法であり，切除の可否について十分検討する必要がある．切除不能例の多くは薬物療法が適応となるが，全身状態の高度低下例，減黄不良例などは緩和治療が適当である．放射線療法あるいは化学放射線療法は遠隔転移のない高度局所浸潤を認める胆管癌で行われることがあるが，位置づけは確立していない．

　薬物療法の1次治療は，ゲムシタビン単独治療（ゲムシタビン療法）とゲムシタビン＋シスプラチン併用療法（GC療法）によるランダム化比較試験の結果^1,2），GC療法が標準治療として位置づけられている．しかし，2011年5月の時点で，シスプラチンは胆道癌に保険適用が承認されていない．現在，上記のランダム化比較試験の結果に基づき公知申請が行われており，早期の承認が待たれている．
