食道・胃静脈瘤に対する内視鏡治療の現状

【ポイント】

◆食道静脈瘤においてEVL治療は，EISに比べ再発率は高いが副作用が少なく，十分なフォローアップと追加治療により出血予防には効果的な治療である．

◆胃静脈瘤出血に対しては，シアノアクリレート（ヒストアクリル^®）の内視鏡的塞栓術は有効である．

◆胃静脈瘤の待機例と予防例には，わが国で開発されたB-RTOは低侵襲で治療効果の高い治療手技である．

◆内視鏡治療抵抗性の食道・胃静脈瘤に対しては，脾摘やHassab手術が有効な症例がある．
