【ポイント】

◆直腸・肛門管癌の隣接臓器浸潤の診断にはCTやMRIが有用であり，術前に合併切除の必要性を検討して適切な治療戦略を立てる必要がある．

◆T4直腸・肛門管癌に対する手術は，癌に対する根治性を保ち，QOLを低下させない過不足のない適切な切除を計画し，実行する．

◆放射線化学療法の目的としては，局所再発率の低下があるが，放射線による機能障害も考慮して適応を決定するべきである．
