【ポイント】

◆T4肝癌の根本的治療法は外科切除のみだが，さらなる成績の向上には補助療法の確立が至上命題である．

◆遠隔転移のない肝機能良好症例に対しては，姑息的治療選択の前に，肝切除を中心とした集学的治療の可能性を肝癌治療専門チーム主導で考慮することが第一と考える．

◆脈管処理を伴う葉切除以上の手術となるため切除可能性の判断と高難度手術に対する技術の裏付けがきわめて重要である．
