【ポイント】

◆胆道癌は，解剖学的位置関係から病状の進行に伴い多彩な進展様式をとり，発見された時点で隣接臓器への浸潤を有するT4胆道癌であることも多く，その診断と治療には専門的な知識や経験が要求される．

◆近年，MDCTを中心とした各種画像診断装置の解像度や解析機能の向上により，腫瘍の局在や進展範囲，胆管の走行や閉塞部位の同定などが可能となり，術前の胆道ドレナージや術式のプランニングにきわめて有用である．

◆周術期管理の向上により術後の合併症の頻度は減少し，進行胆道癌に対して根治切除のための血管合併切除を含む積極的な拡大切除などが施行され，今後は新規抗癌剤による化学療法を含めた集学的治療により，胆道癌の治療成績のさらなる向上が期待される．
