【ポイント】

◆肺癌におけるT4浸潤臓器には，心臓，大血管，気管分岐部，胸椎および食道が含まれ，大血管に含まれるものとして大動脈，上下大静脈，主肺動脈，心膜内部における左右の肺動静脈がある．

◆過大侵襲を伴うT4肺癌に対する手術は，それらの浸潤臓器の特性を十分把握しておくことが重要である．

◆高度な手術手技やチームワークに加え，術前診断の精度の高さが手術成績に大きな影響を与える．
