胸部CT上すりガラス様陰影（GGO）を呈した乳癌肺転移の1例

要旨

症例は55歳，女性．左乳癌に対しCAF療法を4コース施行後，2007年8月に胸筋温存乳房切除術，腋窩リンパ節郭清を施行した．病理所見で浸潤性乳管癌（硬癌），n（10/13），ER（＋），PgR（－），HER2（3＋）で病期はT3 N1M0 stage ⅢAであった．術後にCAF療法を2コース後，胸骨傍リンパ節領域，鎖骨上窩リンパ節領域，胸壁に計50Gy照射し，アナストロゾールを内服していた．2009年2月の胸部CTで両肺に多発するすりガラス様陰影（ground glass opacity：GGO）と肺門，縦隔リンパ節腫大を認めたため，経気管支的肺生検を施行した．病理所見で腺癌の増殖を認め，TTF-1（－），ER（＋），PgR（－）であり乳癌肺転移と診断した．トラスツズマブ，ドセタキセル併用療法によりGGOは消失した．今回われわれは，胸部CT上GGOを呈した乳癌肺転移を経験したので，文献的考察を加え報告する．
