【ポイント】

◆膵癌手術における上腸間膜動脈周囲神経叢郭清は術後の下痢の合併が多く，ときに難治性であり，QOLの低下を招く．

◆術前・術中検査で神経叢浸潤を含め，膵癌の進展を正確に評価し適切な術式を決定することが重要である．

◆術後の下痢に対しては薬物治療を中心とした適切な治療がQOLの向上および栄養状態の改善に重要である．
