【ポイント】

◆直腸手術の際は，疾患の良・悪性にかかわらず術後の排便障害は不可避の問題であり，その病態の理解と対策を熟知しておく必要がある．

◆肛門機能障害の軽減のためには新直腸再建法が重要であり，J型結腸囊再建やtransverse coloplasty pouchなどが用いられている．

◆術後に排便機能障害が認められた場合は薬物療法が第一選択であり，約7割の患者に有効である．
