【ポイント】

◆これまで大腸癌での術中病理検査は，腹膜播種の有無，遠隔転移やリンパ節転移の確認，他臓器浸潤や断端の検索のために行われてきた．

◆Sentinel node navigation surgeryを取り入れ，センチネルリンパ節を術中病理診断することは，郭清範囲の決定，合理化，予想外のリンパ節転移の検索，微小転移の検索に有用である．

◆免疫染色やRT-PCRで術中生検したセンチネルリンパ節の微小転移を重点的に調べることで，再発リスク，術後補助化学療法の適応などに利用できる可能性があり，今後の検討に期待する．
