【ポイント】

◆内科的治療に抵抗性の高度肥満症に対する外科的治療は，欧米のみならず，アジアを含め世界的に急速に増加しつつある．

◆特に消化管の経路の変更を伴う術式では劇的な代謝改善効果が得られるため，metabolic surgeryという新しい概念が注目されている．

◆日本でも年々普及しつつあるが，効果と安全性のバランスに配慮し，日本人に適した術式と適応基準について十分な検討が必要である．
