【ポイント】

◆膵癌の手術適応の決定には的確な病期診断が重要であり，正確な進展度と遠隔転移の有無を診断できるモダリティが要求される．

◆膵癌の進展度診断はCTが中心であり，MPR像やVR像などの多彩な画像構築によって低侵襲に質的診断が可能である．

◆小膵癌の描出にはEUSが他のモダリティよりも優れる．膵管狭窄・拡張のみで腫瘍の描出ができない場合にはEUSを積極的に施行すべきである．
