はじめに

　自家蛍光電子内視鏡とは，内視鏡下に励起光を照射して，生体組織内にある内因性の蛍光物質からの自家蛍光を捉え，電気処理した画像をモニター上に擬似カラー表示するものである．Autofluorescence Imaging（AFI：オリンパス・メディカル・システムズ）は内視鏡下に青色の励起光（照射波長：395～475nm）を照射して生体組織内にあるコラーゲンなどの蛍光物質からの自家蛍光を捉え（検出波長：490～625nm），モニター上に擬似カラー表示する（図1）．当院では通常の上下部内視鏡検査や早期胃癌の内視鏡治療の前に，白色光，narrow band imaging（NBI），色素散布にAFIも適宜組み合わせて観察を行っている．上部消化管においては主に早期癌のスクリーニングや，広がりを精査する際に使用する1つの手段として用いている．

　本稿では，われわれが施行している上部消化管におけるAFI観察の実際を述べる．
