虫垂粘液囊腫に対し腹腔鏡補助下回盲部切除術を施行した4例

要旨

虫垂粘液囊腫は稀な疾患であり，破裂することで腹膜偽粘液腫をきたす可能性があるため手術適応とされる．われわれは虫垂粘液囊腫に対し腹腔鏡補助下摘出術を施行した4例を経験した．年齢は53～79歳で，男性が3例であった．主訴は検診での便潜血指摘が2例，腹痛が2例であった．下部消化管内視鏡検査では3例で盲腸に外圧排所見を認めた．CTでは1.6～3cm径の囊胞性腫瘤を認めた．全例に腹腔鏡補助下回盲部切除術を施行し，腫瘤破裂や術後合併症を認めた症例はなかった．病理組織学検査では粘液囊胞腺腫が3例，過形成が1例であった．いずれの症例も再発を認めていない．わが国での44例の腹腔鏡下摘出例と合わせ考察した．
