【ポイント】

◆Vienna分類はクローン病の病型，病変部位をできるだけシンプルに分類するために1998年に提唱された分類で，診断時の年齢，病変部位，狭窄や穿通などの病態からなる．

◆腹腔鏡下手術の適応については，上述のVienna分類を用いると，B3L3/L4以外，特にB2L1は最も良い適応である．B3L3/L4では，開腹移行率が高率で，合併症もそれ以外の群に比して高くなる．

◆腹腔鏡下手術の長期予後について，開腹手術とのRCTの結果や筆者らの検討では，累積再手術率は開腹手術との間に有意差を認めなかった．
