胆囊炎に併存し診断に苦慮した肝炎症性偽腫瘍の1切除例

要旨

症例は66歳，男性．弛張熱と上腹部痛を主訴に入院した．腹部超音波，CT，MRI検査にて胆囊頸部の壁肥厚とともに，肝S4に18mm径の境界明瞭で内部不均一な腫瘤影を認めた．抗菌薬投与でも症状改善が得られないため，準緊急で胆囊摘出術ならびに肝切除術を施行した．病理組織学的には肝腫瘤内に悪性所見はなく，多核巨細胞や形質細胞を含む炎症細胞浸潤を認めた．免疫組織染色ではIgG4陽性形質細胞が高頻度に出現しており，fibrohistiocytic typeのIgG4関連肝炎症性偽腫瘍である可能性が示唆された．術後は特に問題なく経過し，術後8か月目の現在，外来通院中である．
