腸重積にて発症した小腸原発悪性ラブドイド腫瘍の1例

要旨

症例は59歳，男性．2007年5月に腹部膨満感，嘔吐が出現した．前医を受診し，腸閉塞の診断でイレウスチューブを挿入したが改善せず，当院へ紹介となった．腹部CTで腸重積を疑い，緊急開腹手術となった．Treitz靱帯より190cmの部位で小腸腫瘍を先進部とする腸重積を認め，小腸部分切除術を施行した．肉眼的には小腸内に7cm大の弾性軟な腫瘤を認め，病理組織学的には大型で未分化な異型の強い腫瘍細胞が充実性に増殖しており，小腸原発の悪性ラブドイド腫瘍と診断した．術後約4年が経過するが，無再発生存中である．腸重積にて発症した小腸ラブドイド腫瘍は検索しえた限りでは自験例が初めてであり，ここに報告した．
