食道癌の疫学

◆食道癌の罹患数は年々，増加傾向にある．年齢調整罹患率の年次推移は男性では1975年から緩やかに増加し，女性では横ばいである．年齢調整死亡率の年次推移は男性でほぼ横ばい，女性で減少傾向である．

◆男女比は6：1で，占拠部位は胸部中部食道が51.6％と最も多く，組織型は扁平上皮癌が92.9％であり腺癌は少ない．同時性重複癌は7.4％，異時性重複癌は10.2％と報告されており重複臓器としては胃，頭頸部，大腸，肺の順に多い．

◆国際比較において年次別年齢調整罹患率では男性の場合，1998～2002年の5年間の推計に限って比較するとアジア地域（日本，中国）やフランスで高い傾向があるが，これらの地域も年次推移ではいずれも横ばいから減少傾向である．年次別年齢調整死亡率は1999～2003年の5年間の推計で，男性では28か国中5位で年次推移では横ばい，女性は28か国中14位で年次推移では減少傾向である．
