胆道癌の診断

◆胆道癌診断のファーストステップは血液生化学検査と腹部超音波検査であり，セカンドステップとしてはCT，MRI（MRCP）があり，その後にEUSやERCP，ERCP下での細胞診，組織診，さらには胆管癌であれば胆道鏡検査とステップアップしていく．

◆胆道癌の肉眼分類は乳頭型，結節型，平坦型（それぞれ膨張型と浸潤型に亜分類）その他に分類され，欧米では，papillary type，nodular type，sclerosing typeに分類される．

◆胆道癌の病期分類には，わが国では「胆道癌取扱い規約」による分類が，国際的にはUICCのTNM分類が広く使われている．

◆肝外胆管癌の長期予後解析で，in situ様の粘膜内での腫瘍組織の遺残の有無は患者の術後の生存予後には影響しないが，断端部位での浸潤像を示す癌の遺残は術後の局所再発および術後の生存予後に有意に関連する．
