完全腹腔鏡下胃全摘術における組立式タバコ縫合器を用いたRoux-en-Y再建

【ポイント】

◆組立式タバコ縫合器（Detachable ENDO-PSI鉗子）を用いることによって，開腹手術と同様の手順でcircular staplerを用いた再建が完全腹腔鏡下に可能となる．

◆タバコ縫合器でタバコ縫合をかけたのち，食道を完全に切離せずに前壁を約1/2周切開し，後壁を残すことでアンビルヘッドの食道への挿入が容易となる．

◆完全腹腔鏡下手術では癒着が少ないので，内ヘルニアの予防目的にY脚腸間膜と輸出脚腸間膜のギャップとPetersen's defectの閉鎖を行うことが推奨される．
