腹腔鏡補助下胃全摘術の検討―短期成績と医療経済面からみた開腹胃全摘術との比較

要旨

【目的】腹腔鏡補助下胃全摘術（LATG）を短期成績と医療経済面から開腹胃全摘術（OTG）と比較検討した．【対象と方法】当科で同期間に施行したLATG 20例とOTG 15例を比較した．【結果】LATG群で手術時間は長く，出血量は少なかった．排ガス開始日や歩行開始日，持続硬膜外麻酔期間はLATG群で短かった．術後在院日数では差を認めなかった．手術請求額はLATG群で高額であったが，入院総請求額は同等であった．LATG群で手術経費は高額であったが，手術請求額との差額は同等であった．【考察】LATGはOTGと比べて低侵襲であり，経済面においても不利益とはならず，有用な術式であると考えられる．
