穿刺吸引細胞診で診断しえた乳腺腺様囊胞癌の1例

要旨

乳腺の腺様囊胞癌は稀で，特徴的な細胞像を有する．今回，術前の穿刺吸引細胞診で診断しえた乳腺腺様囊胞癌を経験したので報告する．症例は50歳，女性．右乳房に硬結を触知し，マンモグラフィおよび超音波検査では充実性の腫瘤として認められた．穿刺吸引細胞診では，粘液状物質を取り囲むように腫瘍細胞が集塊をなし，典型的な腺様囊胞癌の細胞像であった．乳腺腺様囊胞癌の診断で乳房温存術を施行し，組織学的にも真の腺管と偽囊胞から成る特徴的な像がみられた．乳腺原発の腺様囊胞癌は予後が良好であるため，その特徴的な細胞像を理解することは，術前診断の精度向上と適切な治療の選択に有用であると考えられる．
