膵癌における腹腔洗浄細胞診陽性例に対する手術適応

要旨

膵癌治療において外科的切除が最も有効な治療法であることは周知の事実である．切除不能例においては予後不良であり，近年，膵癌に対する化学療法の進歩により治療成績の向上が報告されているが，手術療法を凌駕するには至っていない．膵癌においては腹膜播種を伴うものはもとより，腹水細胞診陽性例においても一般的には手術適応とはされていない．当院において過去，膵癌に対し根治手術を施行した症例のうち，腹腔洗浄細胞診陽性例に対する手術症例（2000年以降の5例）に対し再検討を行った．その結果，明らかな腹膜播種を認めない腹腔洗浄細胞診陽性例で，かつ膵体尾部切除にて手術的に局所切除が可能な膵癌症例において，積極的な手術が予後の向上に有効である可能性が示唆された．
