高度肥満患者に対する腹腔鏡下手術におけるポートサイト閉鎖―クローズシュアーシステム^®の使用経験

要旨

腹腔鏡下手術後特有の合併症としてポートサイトヘルニアがある．肥満はそのリスクファクターであり，高度肥満患者では，腹膜前脂肪織に小腸が嵌頓するRichterヘルニアも報告され，ポートサイトの全層閉鎖が望ましいと考えられる．一方で，高度肥満患者では腹壁が厚いため，ポートサイトの全層閉鎖が困難なことが多い．われわれは，体重221.6kg，BMI 64.1kg/m²の高度肥満患者に対して腹腔鏡下袖状胃切除術を行い，クローズシュアーシステム^®を用いてポートサイトを容易に全層縫合することが可能であった．近年，増加傾向にある肥満患者に対する腹腔鏡下手術におけるポートサイトの閉鎖に有用であると考えられる．
