外科的治療と術後化学療法で長期生存を得た横行結腸間膜原発extragastrointestinal stromal tumor（EGIST）の1例

要旨

患者は58歳，男性．腹痛を主訴に前医を受診した．腹部CT検査で左上腹部に30×20cm大の巨大な腫瘍を認めた．上下部消化管内視鏡検査では異常はなかった．腹部血管造影検査で，左結腸動脈から栄養される腫瘍濃染像を認めた．Gastrointestinal stromal tumor（GIST）を疑い，開腹手術を施行した．腫瘍は横行結腸間膜から発生し，消化管との連続性は認めなかったため，腫瘍の完全摘出術を行った．病理学的検査では，HE染色で類円形腫瘍細胞が増生し，免疫組織学的染色でKIT陽性，MIB-1 index（10％）であり，横行結腸間膜原発extragastrointestinal stromal tumor（EGIST）と診断した．術後1年間，メシル酸イマチニブを投与した．現在，術後3年が経過したが，無再発生存中である．
