食道癌手術後に発症した後天性血友病の1例

要旨

後天性血友病は成人に突然発症する疾患で，第Ⅷ因子に対する自己抗体が産生され重篤な出血傾向が出現する稀な疾患であり，悪性疾患などに合併する．患者は65歳，女性で，食道癌根治術後の約1か月目に筋肉痛と広範な皮下出血，血尿で発症した．APTTが65.9秒と著明な延長を認め，正常血漿との混合試験で上に凸のパターンを示したため，ステロイドパルス療法と，新鮮凍結血漿による凝固因子補充療法を開始した．のちに第Ⅷ因子活性低下，第Ⅷ因子インヒビターの存在が明らかとなり，後天性血友病と診断した．経過は良好で，ステロイドからの離脱が可能であった．悪性疾患術後の原因不明の出血傾向の原因として本疾患を鑑別に挙げることが必要である．
