有茎性肝外性に発育したため胃GISTと術前診断された肝原発褐色細胞腫の1例

要旨

有茎性に肝外性発育したため胃GISTと術前診断していた，副腎外としてはきわめて稀な肝に原発した褐色細胞腫を経験した．症例は66歳の女性．左乳癌に対する胸筋温存乳房切除術後のフォローCTで，偶然に胃の近傍に充実性腫瘍（GISTの疑い）を認めていた．術後経過観察中に血中CEAが漸増してきたため，FDG-PET/CTを施行したところ，当該の腫瘍のほかに甲状腺左葉にも集積亢進を認め，甲状腺癌合併と診断した．根治的甲状腺左葉切除の2か月後に腹腔鏡下手術を企図したが，術中に本腫瘍が茎をもって肝縁から肝外性に発育していることから胃GISTは否定され，摘出後の病理学的検索によって“褐色細胞腫”と診断された．本症例は，副腎外褐色細胞腫としてはきわめて稀な肝に原発したものであり，かつこれが有茎性肝外性に発育していたこと，さらに乳癌と甲状腺癌を合併していたことが特筆すべき点であった．
