胆囊癌手術の温故知新

【ポイント】

◆T1胆囊癌は診断さえ正確であれば単純胆摘術で治癒しうる．したがって，術前の深達度診断の精度を上げることが単純胆摘術の適応を決定するために必要である．

◆T2胆囊癌に対する標準術式は，胆囊床肝切除と2群リンパ節郭清（D2）を行う術式である．これに胆管切除を付加するか否かは施設によって方針が異なるが，ルーチンに胆管切除を行うことを支持するエビデンスはない．

◆T3・T4胆囊癌に標準術式はなく，個々の症例でR0手術を達成するための術式を立案しなければならない．そのためにはMDCTを主軸とする画像診断で進展様式や浸潤臓器を正確に把握する必要がある．
