当科における直腸癌局所再発症例の治療成績と集学的治療戦略

要旨

1994～2008年に自治医科大学消化器一般外科で根治手術が行われた直腸癌541例中，局所再発した23例の治療成績を解析し治療戦略を考察した．10例（43%）に再発巣切除が施行されていた．切除例は非切除例より再発後生存日数が延長していた．再発形式を3 type（A：骨盤壁に浸潤，B：骨盤内臓器浸潤，C：吻合部近傍）に分類すると，切除率はそれぞれ15%，71%，100%で，生存期間もtype A，Bに比べCが有意に長かった．よってtype Cは手術を中心とした治療，type Bは手術と全身化学療法，type Aでは全身化学療法に重点を置いた治療が必要と考えられた．また，治療成績の再発type別検討は治療戦略の考察に有用と思われた．
