肛門手術における前立腺肥大症と術後尿閉の関連

要旨

〔目的〕肛門手術の合併症の1つとして術後尿閉はよく知られている．近年，整形外科の領域でその発生には前立腺肥大症の程度（国際前立腺症状スコア：IPSS）が関与するとの報告があった．そこで，われわれは痔核手術後の尿閉について検証した．〔対象と方法〕痔核結紮切除術を施行した170症例を対象とし，後ろ向きのケースコントロール研究を行った．検討項目はIPSS，QOL，熟練度，年齢，Goligher因子，LE（ligation ＆ excision）個数，麻酔薬である．〔結果〕多変量解析の結果，LE個数とIPSSが術後尿閉の有意なリスク因子であった．〔結論〕前立腺肥大症の程度が術後尿閉の発生に影響を及ぼす可能性が示唆された．
