総胆管結石症

はじめに

　総胆管結石症は胆石症全体の約20%を占めているとされる^1）が，いったん急性胆管炎を発症すると重篤な状態になることもあるため，その診断と治療は極めて重要である．また，無症状胆囊結石症と違い，無症状総胆管結石症に対しては，胆石症診療ガイドライン^2）にはいずれ起こりうる胆管炎を見据えた治療の必要性をグレードAとしている．

　その胆石症診療ガイドラインにおける胆石症診断フローチャート（図1）には，

・腹部X線単純写真

・腹部超音波検査（US）

・腹部CT検査

・点滴静注胆囊胆管造影法（DIC-CT）

・MRI検査（MRCP）

・内視鏡的逆行性胆管膵管造影（ERCP）

・超音波内視鏡検査（EUS）

・管腔内超音波検査法（IDUS）

を診断モダリティとしている．

　これらのモダリティを用いて診断し，胆石症診療ガイドライン総胆管結石症治療フローチャート（図2）に基づいて日々治療を行っている．
