はじめに

　膵管内乳頭粘液性腫瘍（IPMN）は膵管上皮に発生した腫瘍が粘液を過剰に産生し，膵管拡張による多房性囊胞性変化などの特徴的な臨床像を呈する疾患であり，通常型膵癌と比較し緩徐に発育・進展し予後は比較的良好である．2006年に国際診療ガイドライン^1）が刊行され，IPMNの治療方針が示されるとともに，広く認知されるようになった．さらに2012年改訂版^2）では，最新の知見に基づき診断基準や手術適応などに一部変更が加えられた．

　本稿では，IPMNの診断と術式決定に際ししばしば問題となる主膵管内進展と併存膵癌に注目し，膵切離の実際と術中膵管洗浄細胞診について主膵管型IPMNを中心に概説する．
