局所進行がん―閉塞症状にバイパスは有用か

素朴な疑問

　切除不能の胃がんや膵がんで胃や十二指腸が閉塞したときは，「食べられるようにする」という理由でバイパス手術を行うが，期待したほど食べられなかったり，腹膜播種で再入院したり，予想より早く死亡したりすることがある．胃十二指腸閉塞を生じた進行消化器がん患者のバイパス手術は本当に有用だろうか．ステントを留置したほうがよいのだろうか．
